Do you suffer with any of these symptoms?

-Cold hands and feet that never warm up.
-Bloating after small meals.
-Low appetite or feeling full quickly.
-Constipation, sluggish digestion, or alternating gut symptoms.
-Morning puffiness in the face and eyes.
-Brain fog, fatigue, low mood, and poor stress tolerance.

These symptoms are often early signs of impaired thyroid hormone activity, even when standard thyroid labs appear normal.

Many women experiencing this are told their thyroid is fine. TSH is in range. T4 looks adequate. Sometimes T3 is even labelled normal.

Yet their body tells a different story.

In many cases, this pattern is not true hypothyroidism. It is Low T3 Syndrome, also known as Euthyroid Sick Syndrome, where the thyroid gland itself is healthy but thyroid hormone is not being converted or used properly at the tissue level.

Low T3 syndrome describes a metabolic state where the thyroid gland is intact and responsive, but the body reduces conversion of inactive T4 into active T3 under stress.

The word euthyroid simply means the thyroid gland itself is not diseased. It is producing hormones and responding to TSH. The problem lies in hormone metabolism, not hormone production.

This phenomenon is well described in the medical literature and is often grouped under Non Thyroidal Illness Syndrome, particularly in states of chronic stress, illness, inflammation, or under eating (Warner and Beckett, 2010, PMID: 20016054).

Why Thyroid Hormone Conversion Matters More Than TSH

The thyroid gland primarily produces T4, which is a storage hormone. T4 must be converted into T3 before it can meaningfully support metabolism, body temperature, digestion, mood, and energy production.

This conversion occurs throughout the body, with the liver responsible for the majority of T4 to T3 activation. Specialized enzymes called deiodinases regulate this process.

D1 and D2 support conversion into active T3.
D3 converts T4 into reverse T3, an inactive hormone that blocks thyroid receptors.

In a healthy, nourished, low stress body, most T4 is converted into T3. Under stress, the body deliberately shifts this balance.

Reverse T3 and Chronic Stress

Reverse T3 is not a mistake or abnormal byproduct. It is a protective mechanism.

When the body perceives threat, such as illness, inflammation, fasting, calorie restriction, or chronic psychological stress, it slows metabolism to conserve energy. One way it does this is by increasing production of reverse T3 instead of active T3.

Human studies consistently show that during fasting and starvation, T3 levels fall while reverse T3 rises, even when TSH remains relatively unchanged (LoPresti et al., 1991, PMID: 1986011).

This explains why symptoms like cold extremities, constipation, bloating, low appetite, fatigue, and puffiness often worsen with dieting or prolonged restriction.This state is profoundly anti-thyroid.

Blood Sugar Instability Suppresses T3 Conversion

Poor blood sugar regulation is one of the fastest ways to impair thyroid hormone activation.

Skipping meals, under eating carbohydrates, relying on caffeine, or pushing through long gaps without food increases adrenaline and cortisol. These stress hormones directly suppress T4 to T3 conversion and promote reverse T3 production.

Human research shows that dietary carbohydrate availability plays an important role in regulating T3 production. When carbohydrates are restricted, T3 falls even in otherwise healthy individuals (Spaulding et al., 1976, PMID: 1249190).

This is why thyroid symptoms often overlap with anxiety, shakiness, digestive issues, and poor stress tolerance.l” labs.

The Liver’s Role in Thyroid Hormone ActivationWhy T4 Therapy Often Fails in Insulin Resistance

Approximately seventy percent of T4 to T3 conversion occurs in the liver. This makes liver health essential for a functioning metabolism.

When liver function is impaired, T4 accumulates without being efficiently converted into T3. This has been clearly demonstrated in patients with liver disease, who show reduced peripheral conversion despite normal thyroid hormone production (Nomura et al., 1975, PMID: 1159078).

More recent research continues to highlight the liver’s central role in thyroid hormone regulation through deiodinase activity and glucose handling (Bruinstroop et al., 2023, PMID: 36892852).

Chronic stress, estrogen dominance, inflammation, nutrient deficiencies, poor bile flow, and long term under eating all reduce the liver’s ability to activate thyroid hormone.

Carbohydrates Are Not the Enemy of Thyroid Health

One of the most damaging myths in thyroid care is that people should avoid carbohydrates.

In reality, adequate carbohydrate intake is essential for T3 production.

Eating carbohydrates, especially fruit and easily digestible sugars, allows the liver to:

  • Replenish glycogen
  • Reduce cortisol
  • Restore proper T4 → T3 conversion
  • Improve insulin sensitivity over time

Low-carb diets, prolonged fasting, and calorie restriction often worsen hypothyroidism, particularly in women.

Low T3 syndrome explains why someone can have normal TSH, normal T4, and sometimes even normal serum T3, yet still experience clear hypothyroid symptoms.

At the tissue level, thyroid signaling is being suppressed as part of a metabolic survival response.

The body is not broken. It is adapting.

The Takeaway

If you have thyroid symptoms but normal labs, the problem may not be your thyroid gland.

Low T3 syndrome reflects a body that does not feel safe enough to run a high energy metabolism.

Cold hands and feet, digestive slowdown, bloating, low appetite, puffiness, fatigue, and brain fog are not random. They are signals that metabolism has been intentionally slowed.

True thyroid recovery begins by restoring the conditions that allow T3 conversion to resume. Adequate fuel. Stable blood sugar. Liver support. And a nervous system that is not constantly in survival mode.
